 I, the undersigned parent or legal guardian of the above-named participant, understand and acknowledge that participation in baseball and related physical activities involves inherent risks, including, but not limited to, injuries such as sprains, fractures, concussions, heat-related illnesses, or even more serious injuries.

I acknowledge that my child is voluntarily participating in the Baseball & Jesus Camp (hereafter referred to as “the Camp”), and I agree to assume all risks associated with participation, including any risks arising from negligence on the part of the organizers, volunteers, staff, or sponsors.

I, for myself and my child, and our respective heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless the Camp, its directors, staff, volunteers, organizers, affiliated churches or ministries, and any property owners where the Camp is held, from and against any and all liability for injury, illness, loss, or damage to person or property incurred while participating in the Camp.

In the event of an emergency, I hereby give permission to the Camp staff or its representatives to secure necessary medical treatment for my child. I understand that every reasonable effort will be made to contact me prior to such action. I assume full responsibility for the cost of any medical care provided.

I give permission for my child’s image, likeness, or voice to be recorded and used in promotional materials for the Camp, including photos, videos, and social media posts. 

I understand that this camp includes Christian-based teachings, devotionals, and prayer. I acknowledge and agree to my child’s participation in these spiritual components as part of the overall camp experience. I also understand that appropriate behavior is expected at all times, and the Camp reserves the right to remove a participant for misconduct.
